If the Google Play listing isn't showing for you, the same steps work with any other M3U or Xtream Codes player from Google Play; only step 1 changes.

Launch it once so it finishes its first setup. You'll land on an empty channel list — that's expected until step 3.
Go to Settings → General → Playlist. Enter your M3U URL, or switch the type to Xtream Codes and enter the server, username and password your provider gave you.
Go to Settings → EPG and enter your provider's XMLTV or JTV guide link, if they gave you one. This step is optional — channels still play without it.
Back out to the main screen and restart the app so it reloads. Channels appear grouped with logos, and the guide fills in if you added one.
